Integrated Clinical Services provides quality health services for people who experience barriers to accessing care. Our vision is to provide services that are integrated and connected to promote optimal health and well being through interdisciplinary team based care. The Primary Care Division assesses need and develops services that strive to deliver timely access and promote racial equity through culturally appropriate care, health screening, and treatment of chronic illness. A primary focus of our services is on prevention, education, and shared decision making to promote well-being. Clients are seen in geographically dispersed sites throughout Multnomah County. Advanced Practice Clinicians (APC) - deliver clinical care, create shared care plans, provide education and resources within the Health Department and external programs that best meet the client's needs and health goals. APC determines diagnosis and treats acute and chronic conditions, leveraging the resources and skills of an interdisciplinary health care team to improve or maintain health status for a diverse, multicultural client population within a busy, primary care setting.
As an APC you will provide professional, comprehensive specialized nursing services with an emphasis on prevention of disease and promotion of health. The APC establishes a therapeutic relationship with a diversity of patients, including refugees and low-income populations.

As the Advanced Practice Clinician, other duties will include:
Direct Clinical Care:
- Within a mobile care environment, diagnoses and treats acute and chronic medical conditions and diseases according to scope of practice across the lifespan to include conducting a medical history.
- Conducting physical exams, performing treatments such as suturing simple lacerations and wound care, etc.
- Ordering and interpreting diagnostic tests, devising a course of treatment, discussing alternative treatment, risk and benefits of treatment, prescribing and monitoring medications, appropriate counseling of patients and reassessment of the patient at appropriate intervals.
- Provide patient education on disease treatment options, nutrition, exercise, and health maintenance behavior.
- Maintains high quality notes and recommendations for treatment and next steps in the care plan as part of a team-based approach.
Indirect Clinical Care:
- Seeks clinical consultation from other clinicians as needed as part of practice management.
- Provide consultation to other providers, nurses and support staff. Prioritize and direct activities of the care team.
- Coordinates, externally with providers and community based organizations, and internally, with behavioral health, pharmacy, and dental providers to effectively integrate and coordinate care.
- As needed provides coverage for clinical colleagues in their absence.
- Completes patient documentation and manages electronic in-basket according to established policies and procedures.
- Utilizes panel based information technology tools to prioritize and direct proactive care for patients assigned to provider or team panel including preventive health screening and chronic disease care management.
- Follow-ups on utilization issues (ED, hospital admissions) including specialty referrals and consultations.
- Works closely with Mobile Clinic care team members including Dental, medical assessment, case manager and others.
- Participates in Team, Professional, and Health Center-wide meetings.
- Participates in continuing education to maintain licensure, and current, evidence based practice.
Clinic Leadership
- Provides clinic and quality oversight of the lab and immunization work within the mobile clinic program.
- Act as the Immunization Lead for the program following the Oregon Immunization management guidelines and annual training.
- Maintains competency in immunization forecasting, administration, contraindications, and documentation.
- Maintains Clinical Laboratory Improvement Amendments (CLIA) lab waive testing and phlebotomy competencies.
- Adheres to medication management policies and procedures, at the Health Department, state and based on practice licensure levels, to maintain the safe storage and dispensing of medications.

Required Minimum Qualifications/Transferable Skills*:
- Masters degree or equivalent in nursing from an accredited college or university
- Possession of a valid Nurse Practitioner or Physician Associate certificate
- License to practice as a Registered Nurse in the State of Oregon OR License to practice as a Physician Associate
- CPR/BLS certification
- DEA certification
- Driver’s License

- Demonstrated knowledge of the principles, practices and procedures of general medicine and surgery;
- Demonstrated knowledge of methods and techniques of diagnosing and treating a variety of diseases and injuries;
- Ability to effectively screen and gather required information in a professional, culturally competent and customer service oriented manner;
- Ability to motivate clients through discussion and persuasion;
- Ability to communicate clearly and concisely, both orally and in writing;
- Ability to establish and maintain cooperative working relationships with those contacted in the course of work;
- Ability to be flexible and manage multiple priorities;
- Ability to maintain composure and work objectively with clients who may be hostile or verbally abusive.
- Preferred: Spanish-English language skills
