The Opportunity
- Seeking Family Care or Primary Care physician assistants and nurse practitioners.
- Current national certification, DEA, and BLS are required.
- At least one (1) year of primary care experience is required.
- Fingerprinting via DOJ will be required.
- Current CA state license is required.
The Practice
San Bernardino Health Center – San Bernardino, California
Adelanto Community Health Center – Adelanto, California
Ontario Health Center – Ontario, California
- A Federally Qualified Health Center.
- Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker.
- Facility hours for this location are Mon-Fri 8am-5pm.
- Good mix of patients including peds.
- 15-18 patients per day per provider.
- Point of Care testing available at site; minimal labs.
