Job Summary

The Population Health team is seeking a community health worker to assist with the inpatient/outpatient Population Health program, specifically supporting Transition Care Management (TCM) for high-risk patients being discharged from the hospital to home. This position will provide vital assistance to patients through home visits, care coordination, and connection to community resources that address social determinants of health (SDOH) barriers such as transportation, housing, and other psychosocial needs. The community health worker will collaborate closely with multidisciplinary teams to ensure patients receive all necessary healthcare services and resources, with confidentiality and thorough documentation maintained within electronic health records and databases.

This is a hybrid position, with both remote work and on-campus attendance expected.

The ideal candidate will have excellent organizational, communication, and computer skills.

High School/GED/UK provided CHW certification completed in 6 months.
