Job Summary

The Nurse Coordinator Clinical will work with the patient and members of the clinical care team to help navigate their way to timely and quality care and to assist in overcoming any barriers to care. This will include, but is not limited to, connecting them to resources and information to make informed decisions, providing education and emotional support, and coordinating team communications. The NN will assist the patient from initial diagnosis through diagnostic testing, treatment. and follow-up or end­of-life care. Other duties include processing Medicare and high level problem solving.
