Benefits Day 1Role:
The Hospice Resource Advanced Practitioner Registered Nurse (APRN) is responsible to provide pre-admission face-to-face visits for patients referred to the hospice program and telephone coverage during assigned shifts for existing hospice patients. Provides high quality care that is consistent with the hospice plan of care and patient/ family goals of care.
Qualifications:
• Master’s degree in nursing
• APRN certification recognized by CMS for Medicare billing
• Licensure by state of Florida to work as APRN
• Current DEA license
- Current Medicare and Medicaid Numbers
• Active BLS for healthcare professionals from the American Heart Association or Red Cross. Employees hired prior to 12/31/2022 must obtain certificate prior to 10/1/2023
• Knowledge and experience in medical practice and symptom management
• Advanced clinical skills which allow thorough patient assessments and management for any patient with advanced chronic disease as well as for those near the end of life
• Ability to work under protocol with multiple physicians in multiple settings
• Basic computer skills including use of email and word processing (Microsoft Outlook and Word or equivalent programs)
• Strong interpersonal skills, communication, collaboration, and conflict resolution
• Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
• Ability to work independently exercising discretion and independent judgment
• Ability to work in collaboration with other disciplines within the organization
• Certified by appropriate hospice nursing organization within two years of hire date
• Must be able to physically access any home in any area of Company service area, i.e., navigate stairs, and narrow spaces, tolerate heat and lack of air conditioning

Job Responsibilities:
• Provides direct patient care through home visits, as well as clinic/nursing home/assisted living/in-patient unit/ hospital visits, as designated by the Medical Director.
• Works collaboratively with the collaborating physician and the interdisciplinary group, fostering collaborative relationships in the provision of high quality, cost-effective palliative care to hospice patients.
• Handles phone calls for clinical staff; including Hospice House or Hospice Help 24 during assigned shifts.
• Maintains high quality, timely clinical documentation.
• Appropriately prescribes pharmacologic agents to ensure that patient medical needs are met.
• Responds to request of change in patient condition, resulting in level of care change that requires transfer in or out of general inpatient (GIP) setting; hospice house or contract hospital, or need for continuous home care.
• Provides telephone medical coverage for all hospice patients, including those whose non-hospice attending physician desires consultative services or cannot be reached.
• Communicates appropriate clinical information to appropriate person(s) at the completion of shift.
• Leads specific projects as directed by the Staffing Services Manager, Medical Director, or Corporate Medical Services.
• Participates in medical reviews, as needed, and directed by the Medical Director or other Physician.
• Performs other duties as assigned.
