We are in search of a tactical, relationship-oriented, self-starter to join our Provider Relations team. The Provider Relations Representative will serve an important role overseeing our network of home health providers and the markets they operate in. This role’s main function is to build and maintain strong partnerships with various stakeholders in each market, with an emphasis on the home health network. Additionally, the Provider Relations Representative is responsible for addressing provider-driven concerns, education, analyzing and executing market improvement initiatives, and promoting partnerships that align to the company’s strategic mission and programs.

Job Responsibilities:

  • Support and lead Joint Operating Committee (JOC) meetings with participating home health agencies, driving collaboration and innovation that improve outcomes for members and enhance patient and provider satisfaction.
  • Cultivate and maintain positive relationships with the network.
  • Serve as a primary point of contact for provider inquiries, concerns, and collaboration.
  • Provide ongoing education and information to the network regarding company policies, enhancements, initiatives, and programs.
  • Work closely with internal teams and providers to resolve issues raised promptly and professionally.
  • Identify and track common stakeholder issues, providing leadership with recommended resolutions.
  • Create actionable goals centered on improving agency and network performance, innovation, and professional development.
  • Monitor and report out provider performance metrics and key performance indicators.
  • Develop and drive execution of action plans that improve performance-related issues and opportunities to further optimize the network, including provider network expansion efforts.
  • Support the participating provider onboarding process, ensuring the network understands and adheres to company policies and procedures, credentialing, and regulatory requirements.
  • Act as a liaison between providers and internal stakeholders.
  • Carry out network management assignments and other duties as directed by the management team.

Requirements:

  • Bachelor’s degree in Healthcare Administration, Business, or a related field.
  • At least 2 years of managed care or home health care experience in a community liaison, customer service, provider relations, or similar role.
  • Knowledge of home healthcare regulations, compliance, and reimbursement policies.
  • Must be able to diffuse situations and work in a fast-paced environment.
  • Excellent communication and interpersonal skills.
  • Proficient with Microsoft Office (Word, Excel, Outlook, PowerPoint).
  • Willingness to travel quarterly, at minimum.

  • Previous home health experience a plus.
