Position Summary:

Manages and coordinates the prevention, screening and referral program funded by DPH/OHA. Ensure overall programmatic and service delivery compliance. Works closely with Project SHINE leadership and PrEP manager to ensure quality prevention and screening service delivery, tracking, data management, reporting, and contractual follow through. Actively participates in program/service development, required meetings and funder relationship management. Ensures adherence to MDPH/OHA and other funder mandates and guidelines. Oversees daily operations of integrated prevention & screening services and is available to provide direct services to patients. Reports to the Project SHINE Manager.Essential Duties & Responsibilities
  • Coordinates and oversees the Prevention Screening and Referral Services & activities in compliance with DPH/OHA requirements & standards.
  • Ensures adequate coverage for all clinical sessions/ hours of operations by appropriately coordinating staff schedules.
  • Provides Prevention & screening team with supervision, support and guidance.
  • Maintains close communication with Department Manager around PSR activities/initiatives/commitments and work together to address any programmatic/staffing challenges or barriers.
  • Oversees adequate inventory of supplies, educational materials, necessary forms, timely and appropriate delivery of specimens, etc.
  • Adheres to DPH/OHA reporting requirements maintaining accurate and organized documentation of testing data in EPIC, other data base systems.
  • Develops, updates documentation tools, forms, and mechanisms to facilitate data collections
  • Works closely with PrEP Manager on effective and efficient PrEP Navigations workflows/services.
  • Works closely with PrEP Navigators to organize and coordinate in-reach/outreach efforts to specifically include health education and HIV counseling and testing at EBNHC for patients and staff and in the community under the direction of the program manager.
  • Will participate and collaborate as part of the Quality Management and Improvement Team.
  • Provides orientation & training to new Prevention & staff; ensures training manual is assembled.  Reviews integrated HIV/STD/HEP C testing, counseling session content, documentation, specimen collection and policies & procedures, confidentiality, management of EPIC encounters and provider communication.
  • Ensures adherence to MA DPH/OHA specimen collection, handling, packaging, and submission by performing quality control reviews and will properly train staff.
  • Collect and process blood and urine samples for testing. Perform venipuncture, sample collections, specimen handling and processing, submission forms and packaging and shipping process for all samples.
  • Interacts and promotes with departments to increase and channel referrals through the confidential and testing programs.
  • Oversees collaboration with Harvard Medical Students and works closely with designated Health Educator/Counselor around orientation, documentation and training requirements.
  • Responsible for collaboratively working with Program Manager around program planning and development of the testing program to include: expansion to identified EBNHC departments, formal linkages with CBOs/ASOs for provision of mobile unit services, staffing patterns, new initiatives and implementation of comprehensive services etc.
  • Provides input and collaborates with Program Manager on annual performance evaluations of PSR staff.
  • Is actively involved in the planning and coordination around the delivery of HIV positive results ensuring client and staff safety.
  • Accompanies Program Manager to MA DPH/OHA State Mandated Meetings and conferences.
  • Actively collaborates with meeting contractual reporting deliverables & deadlines. (Annual Work plan, Annual progress reports, HIV/STD Reporting, etc.)
  • Review, download and process all tests results from and through Electronic Laboratory Reporting (ELR). Enter and document all the tests and procedures on EPIC.
  • Fill out and send new HIV Case Report to the HIV/AIDS Surveillance Program.
  • Effective and collaborative work with Project Nurse to ensure that individuals testing positive for HIV/STDs/HepC receive treatment and are linked to care.
  • Track positive patients (HIV/STI/HCV),that were referred to treatment or PCP follow up.
  • Report and coordinate partner notification for those new HIV positive patients with Disease Intervention Specialist program (DIS).
  • Prepares and actively participates in monthly call with DPH/OHA contract manager.
  • Completes monthly report in advance of monthly monitoring call.
  • Participates in required departmental activities, events, and meetings
  • Serves as the contact person for mobile unit sites.  Responsible for maintaining working relationships with substance abuse treatment facilities, community-based agencies and schools in CREW area and for scheduling prevention & education groups.
  • Will actively participate in funder monitoring site visits and will address any recommendations and/or findings.
  • Will identify training needs for prevention & screening team.
  • Flexibility to adapt to changes in department needs, scheduling and job content.
  • Performs other duties as assigned.

EDUCATION:

  • BA/BS preferred. 

EXPERIENCE:

  • Community Health Worker Certificate and /or experience providing community health work to culturally, economically, and socially diverse communities, including LGBTQ+ community.
  • Experience working in a team and leading initiatives.

SKILLS/ABILITIES:

  • Good assessment, crisis intervention and counseling skills. 
  • Good understanding of substance abuse and cultural issues.
  • Bilingual and bicultural strongly preferred. 
  • Strong interpersonal skills. 
  • Good verbal and communication skills. 
  • Experience with Microsoft Word and Access. 

PAY RANGE:

$24.15 up to $36.22 based on experience

Salary range

  • $24 - $36 per hour
