LOCATION:T.O. Vinson Health Center
DIVISION/DEPARTMENT: Community Health
SALARY INFORMATION: $36,954.85 Annually / JCCP Code HCP120 / Pay Grade G
Job Responsibilities
Under general supervision facilitates and participates in the evaluation, development, review, of Individualized Family Service Plans for infants and toddlers with developmental delays/medical conditions; assists families in gaining access to early intervention services; manages families’ services received from Babies Can’t Wait; assists in determining the needs of the families and are required to make home visits; makes necessary referrals for other social services; assists in transitioning families to community resources such as local school system and Head Start prior to children aging out of the program.
Minimum Qualifications
Master's degree in a related field from an accredited college or university AND Eligibility to be licensed
OR
Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.
PREFERRED QUALIFICATIONS: Master’s degree in social work, psychology, counseling, sociology, education, nursing, vocational rehabilitation, or a related field and two (2) years of full-time (or equivalent part-time) work experience in social services delivery (case management, outreach, prevention/education, client assessment, or the development/implementation of treatment, rehabilitation, or service plans).
