Job Responsibilities: • Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up. • Coordinates the Plan of Care, ensuring that an individualized Plan of Care is developed that accurately reflects the patient’s evolving needs. • Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process and safety practices. • Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals. • Participates with the IDG to evaluate hospice referrals/admissions for level of care appropriateness. • Attends patient deaths; prepares body for transport; oversees disposal of medications. • Provides appropriate bereavement interventions to the family and caregiver in conjunction with psychosocial staff. • Presents concise and pertinent oral and written reports to IDG; respects and encourages input from all disciplines. • Communicates accurately and completely to physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others. • Supervises patient care given by other nursing team members (i.e., LPNs, Hospice Aides, and contract nurses). • Performs other duties as assigned. JOB STANDARDS:
Minimum Faculty Qualifications
A master’s degree from a regionally accredited college or university and a minimum of 18 graduate semester credit hours (or equivalent) in healthcare management courses is required.
Electronic health records and medical coding certification, training, or experience is preferred.
A minimum of two years of work experience in healthcare management, medical coding, or electronic medical records management is preferred.
Teaching experience, especially in a community college environment, is preferred.

Aspire Health Alliance is seeking a dedicated Clinical Recovery Specialist to join our Adult Community Clinical Services (ACCS) team. Under the supervision of the ACCS Assistant Director, the specialist will provide clinical assessment, risk/safety planning, and treatment planning for adults with serious mental illness. The role is pivotal in leading the development of comprehensive, recovery-based services and coordinating a multi-disciplinary team to evaluate and enhance program services. Key responsibilities include providing clinical leadership for an assigned group of clients, coordinating and monitoring the activities of the individual treatment team, and assuming primary responsibility for the development, implementation, evaluation, and revision of overall treatment goals and plans in collaboration with the team. The specialist will conduct comprehensive assessments of psychiatric history, safety planning for all risk issues, and provide on-call crisis intervention during nighttime hours, serving as a backup to evening and weekend staff. Documentation of client progress is essential to maintain a permanent record of client activity according to established methods and procedures. The role also involves participation in substance use treatment services, communication with guardians, family members, doctors, hospitals, therapists, Human Rights Coordinator, and DMH to coordinate schedules, supports, and services. The specialist will lead in all client intake and assessment processes and explore social and support opportunities for individuals to participate in the community. This is a regular/at-will position with no per diem.
