Effective communication with both patients and healthcare providers is crucial to ensure smooth processing and compliance in these matters.Key Duties and ResponsibilitiesThe key duties and responsibilities of the Forms Coordinator include, but are not limited to:Form Completion<ul><li>Accurately complete and process FMLA forms and related documentation in accordance with federal regulations and clinic policies.</li><li>Ensure all required information is gathered from patients and healthcare providers to facilitate the forms processing.</li></ul>Patient Interaction<ul><li>Communicate with patients to collect necessary information and clarify details needed for all forms.</li><li>Provide guidance to patients regarding the FMLA, Disability and Return to Work process and required documentation.</li></ul>Documentation Management<ul><li>Maintain and organize patient records related to requests.</li><li>Track and follow up on the status of forms and ensure timely submission to employers or insurance companies.</li><li>Tracking and logging incoming forms for timely processing. </li></ul>Regulatory Compliance<ul><li>Stay current with changes in FMLA regulations and ensure that all forms and processes are compliant with federal laws.</li><li>Address and resolve any issues or discrepancies related to FMLA forms and patient records.</li></ul>Administrative Support<ul><li>Assist with other administrative tasks as needed, including scheduling follow-up appointments or coordinating with other medical staff.</li><li>Provide general support to the office as directed by the Office Manager or Practice Administrator.</li></ul>Education/Experience<ul><li>Medical Assistant Certification, or comparable experience in a clinical setting required.</li><li>High school diploma or equivalent required.</li><li>Experience with FMLA, Disability, and Return to Work forms and procedures strongly preferred.</li></ul>Knowledge, Skills and Abilities<ul><li>Demonstrated passion around sports performance and success in supporting multiple orthopedic sub-specialties.</li><li>Previous successful experience working in a clinical setting; experience in the orthopedic specialty is preferred.</li><li>Demonstrated proficiency with Microsoft Office programs such as Excel, Word, and Outlook.</li><li>Demonstrated proficiency with Epic EMR experience preferred.</li><li>Demonstrated knowledge of OSHA and HIPPA regulations and ability to apply those regulations on a day-to-day basis.</li><li>Demonstrated knowledge of medical terminology and ability to educate patients and/or Strong understanding of clinical chart notes and ability to relay clinical information into required forms. </li><li>Excellent organizational skills with the ability to manage multiple tasks and priorities.</li><li>Exceptional attention to detail and accuracy in form completion and documentation.</li><li>Effective communication skills, both written and verbal, with the ability to interact professionally with patients and healthcare providers.</li></ul>Work Environment/Physical DemandsThe work environment/physical demands described here are representative of those that must be met by a teammate to successfully perform the essential functions of this job.&nbsp; Reasonable accommodations may be made to enable differently abled persons to perform the essential functions.
