Position will be working Monday- Friday, with a few evening responsibilities to cover groups.&nbsp;Duties and Responsibilities:<ul><li>Case management – overall coordination of a patient’s care while receiving outpatient services, including communication with family, referral, and other concerned persons., communication with referral sources, assessment of patient progress and communication to the clinical team regarding special treatment needs and procedures.</li><li>The Outpatient Therapist is responsible for facilitating assessments such as level of care evaluation, psychosocial interview/integrated summary and complete all associated documentation and communication with the treatment team.</li><li>Facilitate group and individual therapy that is consistent with patient treatment goals on their treatment plans. Complete all associated documentation.</li><li>Provide family therapy when it is clinically appropriate.
