Multnomah County's Healthy Birth Initiative is a comprehensive program aimed at improving pregnancy outcomes and reducing health inequities for families in the community. Through home visiting programs like the Nurse-Family Partnership, the initiative provides culturally competent care, education, and support to first-time, low-income mothers, with a focus on empowering African American families. By addressing social determinants of health, promoting healthy behaviors, and fostering strong parent-child relationships, the program strives to ensure that every child has a healthy start in life.

***African American Culture KSA (Knowledge, Skills & Abilities): This position requires advanced knowledge of and experience working with the African American community. This would be demonstrated by knowledge of the traditions, values, and customs of the community being served as well as the ability to identify specific examples of experience (professional and lived) working directly with individuals and families who identify with the African American community.

As the Community Health Nurse - African American KSA, responsibilities will include:

Conducting Home Visits

  • Provide home visits to eligible first-time, low-income African American mothers and their families.
  • Adhere to the NFP model of home visitation, following visit guidelines and program standards.
  • Manage a caseload of 25 or more clients.
  • Conduct home visits in a way that is respectful, builds trust, and prioritizes client needs.

Providing Holistic Assessments

  • Assess the physical, emotional, social, and environmental needs of women and their families.
  • Conduct screenings for various health concerns, including intimate partner violence, depression, and child development.
  • Analyze assessment findings to formulate nursing diagnoses and develop individualized care plans.
  • Evaluate client progress toward goals and NFP outcomes, adjusting interventions as needed.

Delivering Culturally Competent Care

  • Demonstrate extensive knowledge of African American culture, history, and experiences, including their interactions with the healthcare system.
  • Utilize a trauma-informed approach, recognizing the impact of historical trauma and chronic stress on families.
  • Provide culturally relevant education, counseling, and support that meets the unique needs of African American families.
  • Build strong, trusting relationships with families based on respect, empathy, and cultural understanding.

Educating and Empowering Families

  • Provide health education and counseling on prenatal care, postpartum health, child development, and parenting skills.
  • Utilize effective teaching strategies based on adult learning theory, motivational interviewing, and principles of self-efficacy.
  • Empower families to set goals, make informed decisions, and advocate for themselves and their children.
  • Connect families with community resources and support systems to address their social service needs.

Salary/Pay:Please be advised that the pay range listed for this position is intended to provide general guidance on the earning potential for the role. However, actual compensation will be determined in accordance with the Oregon Equal Pay Law and will take into account factors such as the candidate's relevant experience and education. Candidates should expect that initial offers will be made within the listed pay range and may not be at or near the top of the range.

Minimum Qualifications/Transferable Skills*:

  • A bachelor's degree
  • At least one (1) year of experience that demonstrates the ability to perform the duties of the position
  • License to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021.
  • CPR certification required.
  • African American Culture KSA: For KSA Cultural positions you must demonstrate your affiliation/membership and experience (lived and professional) working in your African American communities with school age children, youth and families

  • Demonstrated experience working with populations experiencing addiction, substance use, and houselessness.
  • Knowledge of cultures, customs and social experiences of communities experiencing marginalization and barriers to services and opportunities.
  • Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values.
  • Ability to maintain a safe and healthful workplace.
  • Excellent communication and interpersonal skills.
