Community Health Worker

Perinatal Health Partnership

(Covering Butts, Lamar, Pike, Spalding, & Upson Counties)

Under supervision, provides home visitation services to medically diagnosed high-risk, pregnant, post-partum and inter-conceptual women. Works with case managers and other public health nurses and in collaboration with health care providers and other human service agencies to provide person-centered support to individuals and families; to help improve access to care; assist with navigating the health care and social service system; support research through outreach and advocacy and health needs assessments; advocate for individual, family and community needs; and build client capacity to increase knowledge and self-sufficiency. Provides outreach, support, and education to the target population and their surrounding community to reduce disparities in birth outcomes. Daily travel is a requirement for this position.

All District 4 Public Health, County BOH employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Distribution site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.

Job Responsibilities

  • Performs home visits and completes required documentation per the Perinatal health Partnership guidelines to eligible women, infants, and children
  • Works with a team to provide case management services to high-risk perinatal women, infants, and children through home visitation
  • Completes quarterly reports
  • Participates in health fairs and educational programs
  • Prepares to respond to public health emergencies and responds as directed
  • Uses computer technology to collect, store, and retrieve data
  • Participates in programs planning
  • Communicates in writing, orally, and through electronic means, with proper terminology and cultural proficiency
  • Incorporates strategies for interacting with persons from diverse backgrounds (e.g., cultural, socioeconomic, educational, racial, gender, age, ethnic, sexual orientation, professional, religious affiliation, mental and physical capabilities
  • Adheres to the organization’s policies and procedures
  • Provides general clerical support
  • Uses individual, team, and organizational learning opportunities for personal and professional development
  • Candidate must have a valid, unexpired driver’s license and reliable transportation
  • Occasional travel throughout the district's 12 counties for training and program support as necessary

Minimum Qualifications

Social Service Tech 3

High School diploma/GED AND three (3) years of job-related experience in a social service-related position; OR One (1) year of experience at the lower-level Social Service Tech 2 or equivalent position

Additional Information

Preferred Qualifications:

  • Public Health experience
  • Experience with screenings, immunizations, GRITS, and interviewing patients
  • Bilingual Spanish/English Speaking Language
  • Home visitation experience
  • Prenatal Care experience working with high-risk mothers and babies
