Your Role: Making Every Moment Matter
As the Director of Home Health, you will be at the heart of our mission, ensuring that every patient receives the highest standard of care. Your leadership and compassion will guide our dedicated team in providing the care that our clients not only need but also deserve.
Your Key Responsibilities:
- Lead with Compassion: Supervise, communicate, and collaborate with our home health team to deliver empathetic and effective care.
- Be a Beacon of Hope: Conduct initial and ongoing assessments and develop personalized care plans that truly make a difference.
- Empower and Support: Provide assistance and training to clinicians, fostering a nurturing and supportive environment.
- Strive for Excellence: Lead quality assurance and improvement activities to continuously enhance our services.
Your Qualifications:
- A current Registered Nurse (RN) license or PT
- Two or more years of experience as an RN in healthcare or as a PT, preferably in home health or hospice.
- Proven supervisory experience.
- Comprehensive knowledge of applicable local, state, and federal regulations.
