The Occupational Therapist (OT) conducts in-home evaluations and occupational therapy evaluations with PACE participants in coordination with the interdisciplinary team. The OT develops and administers a plan of care according to their scope of practice. Supervises clinical aspects of COTA, and rehab aides (as needed) and is utilized across all PACE sites and in the PACE community. They communicate and collaborate with the Interdisciplinary Team in developing the care plan for each participant, based on OT assessments. Automobile required.Essential Duties & Responsibilities:
- Responsible for initial OT evaluations, reassessments and the development of a treatment plan which is consistent with participant needs.
- Determines participant’s mental and cognitive status, and takes into account psycho-social issues pertaining to occupational therapy program.
- Completes Home safety Assessments for initial, annual, and change in status.
- Identifies DME in the home, and follows policy to log inventory and /or repair/replace damaged equipment.
- Acts as an advisor to the Recreation staff in planning and implementing the physical activity program for participants.
Qualifications and Requirements:
- Bachelor degree in occupational therapy required.
- Must maintain valid license to practice occupational therapy in Massachusetts.
- Minimum of 2 years geriatric experience in health care or community services setting. Home health and outreach or marketing experience preferred.
- Must be able to travel to referral sites and make visits to prospective enrollees in a variety of home and institutional settings.
