Job Summary

The Ambulatory LCSW will serve as a member of a multidisciplinary health care team to provide therapy and social services to patients and families within the outpatient setting.

This will include completing and documenting psycho social assessments to determine the needs of the patient, providing direct behavioral health counseling for patients with mental health illness and/or psycho-social stressors, providing direct counseling and education to patients and families regarding their eligibility for resources and services in the community, and formulating and implementing outpatient plans with the rest of the multidisciplinary patient team.

Position is 100% remote, with all appointments being performed via telehealth.
