As the nurse you will work directly with clients that live in residential group homes as well as clients who reside independently in the community. The RN is responsible for conducting health assessments, assessing health needs, making appropriate referrals to providers, building and maintaining relationships with health service providers in the community to ensure comprehensive continuity of care for clients, and providing ongoing education and support related to client’s medical needs.
